Embedding Page In Website - IE7 Script Error

An embedded form which had been working great on one of my web pages is now generating a script error during page load. I'm also getting this same script error when I try to embed a newly created form. I'm running IE 7 Build 5730. I do have an embedded form that I created in April 07 that seems to still load fine.

I notice that the new Creator application no longer contains the same hyperlink to 'Embed this page in your site' (or if it does I'm not seeing it). When clicked, this use to provide a choice between IFRAME, JAVASCRIPT, and PermaLink code. From what I can tell, with the new Creator app the way you generate code for embedding the form into a web site is by selecting 'More Actions' and then 'Embed in Your Website'. I believe this only generates IFRAME code. I don't see an option for JavaScript. Also, I notice the the IFRAME code generated using the former 'Embed this page in your site' link is different than the IFRAME code generated by 'More Actions' (using the same application form).
