Emebedded forms not rendering correctly after 1st Submit

When I initially embed a form in an external web page it works great, but after an undetermined period of time it stops behaving correctly.

When the page initially loads the form always renders correctly, and the user can successfully submit their first set of values.

However when the embedded form re-renders, it is not of the embeddable type -- it is much too long to fit in the iframe size and the More Actions menu appears in the header of the form, giving the user options in a drop down menu that they should not have in an embeddable form. When they submit using this version of the form, the cogwheel.gif spins indefinitely and though the data was submitted, the form never refreshes.

I have seen this problem occur in several applications. The strange thing is that when the application is first created the embedded form behaves correctly for awhile. It is only after a time that the behavior described above begins.

You can see this behavior if you try and embed the form in the application I have made public, "Seattle Coffee Shops".
