Enquiry Form in Website

I have approx 500 properties on a website and I want to embed an enquiry form that will update my database aswell as email me the enquiry. I would like the received email to be from the enquirer so that I could reply.

Also each property has its own reference i.e VA50, R1030 etc and I need a way of being able to tell which property the enquiry is about. It is easy for me to adjust the html code on each page to suit but I do not know how to do this with a form that is embedded.

Any suggestions greatly received.
NB My website currently has php formmail and i have tried adding the data by email option in Zoho but it does not work directly from the form. My enquiry form is shared with support.
