Error Loading Form - PLEASE HELP!

Hi

When I try to open the form in Zoho it keeps showing a message saying....Loading Form....and the form never shows up

Please...this is urgent, I deleted all scripts, everything, the form as it is right now, only have fields and no scripts but still a no show when I try to open it.

Already shared the app to zoho and gave access. The app is BR-Calc Courier and the form Calculadora

Please HELP
