File upload issues

Hello there guys,

I'm here to report a problem I've been facing with a form I created for my blog. In it theres a upload file line, which doesn't seem to work. All the other data added in the form by users are sentto my email, bot not the attached file.

Someone has a clue of whats happening? I've searched everywhere to try to solve my problem.
