FINISH/SUBMIT Button Missing - Cannot Create New DB

FINISH/SUBMIT Button Missing - Cannot Create New DB

I have 2 databases on http://creator.zoho.com/mcgeepj2/ and am trying to create a 3rd, but I do not see a FINISH or SUBMIT button after pasting my .csv file contents.

I've tried different computers and browsers so I don't think it's a cache problem.
