for each in getting executed twice

Sorry if this has been reported. I feel like I've seen a post like this somewhere before.

My for each x in Form[] loops execute the code inside twice, but only for the last result (or if there is only one result). I didn't notice this behavior before the maintenance that happened a few days ago.
