Form embedded - requiring login?!

I have embedded the from, looks great, works great - if I am logged in. That is helpful, only for me! Guests to my site are seeing the same thing. I am obviously missing something or have overlooked a a step in this process.

I browsed the messages and couldn't find a solution to this.

Any tips....
