Form is not showing up

I had a form created called reliability, and when I added a "look up" field, the form disappeared. I can select it and can edit the script but when I try to view to edit it, the form isnt there (all I want to do is delete it and maybe start over but I cant even get rid of it). Can you help me delete it or get it back.
