we have a form that we've embedded on a web page. we had been accessing the form successfully for some time using Safari on iPhones. Now, suddenly, when we go to the page on the iPhone, the page renders, but the form does not. 

The message in its place is:

"Note: Cookie is disabled in your browser. Please enable the cookie to continue."

1. cookies are enabled.

2. nothing has change in the application/form. we haven't touched it.

3. we get the same result from multiple phones.

any idea what could be causing this failure?
