Form not loading

My application https://creator.zoho.com/sancher/vouchers contains a form called Voucher.
(See attached image of what is seen.)

This used to load perfectly well, till last week.

But suddenly today the form is not loading.
The application contains some reports, which are loading without any issue.

Please correct this issue as soon as possible.

I've allowed edit access to the application to the support team.
