Form stopped working

I just checked my very simple contact form and it stopped working (it worked as early as 5 days ago)...

I didn't make any changes at all but now when I try to submit the form I get the response "Invalid entries found".

I've tried multiple times making sure the verification code is correct. I've tried directly on your site and also the embedded version on my site on multiple browsers and I get the same message.
