Form submission error. XML style missing.

Please see the attached screen shot for the error that I receive when submitting my form.

I'm not sure why. Form submits the information to the correct email notifications. Although, this error screen can make those that submit the form to doubt whether the form went through or not.

Is there an XML file that I need to include in a script somewhere? Do I need to upload an XML style file to my server? Or is this a bug that needs addressing on your end. Can you fix this today? Thanks.

The form is public. It is call CROWN/CDL Quote Request. Application name is Request Quote.
