Getting an error "Invalid entries found. Please correct and submit again." and now changes were made.

I spent the whole day trying to solve the problem. I was creating a new form and I was getting the problem and didnt know why. When I tested over other forms, that used to work normally, and stopped to work for some reason.

I need help urgently.
