Help help!!! Security failure?

Hi

I just discovered that when we type in one of our employees and then write zoho on google - our requestform appear and everybody can make a request to us.

This simply does not work. What can we do to remove this link and make sure that it will not appear again.

Please see attached. If I click the 3th from the top (in red) our form appear and everybody can write to it. Our database is private.
