I'm having a lot of trouble getting my hide/show when a checkbox is selected to work.

 

I click on my checkbox and then go to "On User Edit" Script for that particular checkbox.

 

I click on the edit if statement and insert input.Decision_Box_1 as I've seen previous and i get an error message that says Decision_Box_1 is not declared.  However, when I click to set up a checkbox I can't find anywhere it tells to define field names for individuals checkboxes, only for the entire checkbox area.  Can someone help me with declaring Decision_Box_1 or tell me what I'm doing wrong?
