How can I insert a URL link on a Form - is it possible?

So far loving Zoho, have created a user registration form for our website

But I would like to create a link to the terms and conditions (a PDF) document to open in a new window. This link should either be linked to "Accept Terms & Conditions" or a link below saying "Click Here For Terms & Conditions"
