How do i add more than one phone records for each contact?

How do i add more than one phone records for each contact?

i'd like to add more than one number for each contact, such as work, home, fax...even two work numbers. i prefer to capture these into another form.

The question is: how to i link up the relationships between the two forms, contact & numbers? An example would be very much appreciated.
