How do I choose from 1 dropdown and fill 2 fields.

How do I choose from 1 dropdown and fill 2 fields.

I have a form with a lookup field. It lets you choose a contacts name.  When I choose the contact name from the dropdown list (or after I submit the form) I would like the email address of that contact to also be added.

Can someone help me figure out how to do that?
