How do I create a searchable public view of private data?

I have an app where we create referrals for new business. It is private and noit shared. I want to give public users the ability to enter their last name and see a view of referral details that they made.

Cant figure out how to create this. Is it a foirm to a view or a filtered view?

Please help and provide examples or code.
