Implementing Entity Relationship

Hi all

Im quite new to zoho creator but knowledgeable in using MS Access. Hope you can help me in my problems I have listed below.

I am making a accomplishment reporting system. The following are my entities and their Entity Relationships:

Office which relates to many tasks (One to Many ER).

Each task has many activities (Again One to Many).

Each activities has also many outputs (e.g. training activity that aims to train teachers and develop lesson guides, which is again One to Many ER)

My problem is I dont know how to implement ER in the Report form I am developing.

I tried making one which can be found in this link:

This report form is not capable on implementing one to many ER because you need to submit the form first before you can input again another task for an office or another activity for a task.
