Importing Data

I am trying to import data to my simple database. I tried copying and pasting the data from an .xls and also tried from a cvs file.
Neither worked and all I ended up with was empty records.

There is data in only five of twenty columns but I didn't think this would be an issue?

Can you help please, I don't want to enter 140 records manually.
