Importing xls into existing form or view

I have an application that I use daily. Suddenly, got a problem with one of the forms - it's gives me an error when I try to edit or add records - "There is no such form in Zoho Creator"

So I can't add or edit data, even though I can see the data in the form.

I exported the data to an CSV file and copy the database, figuring I could import the CSV/xls data back into the new database right into the same form in the new, copied database.

There appears to be no way to do this without creating an entirely new form, which I prefer not to do.

Can anyone help?
