I have attached a picture showing in other software the use of a frame to indicate the purpose of 2 or 3 fields.

My second attachment shows similar information on a Zoho Form.  How can I group these fields together logically and cosmetically?  I would like to use a frame or what have you to show the user that the fields (3) are related in purpose.
