I've tried to create a form using this specific excel file but every time i get the following error.

The date format selected does not match with your data. Please select the exact date format.

Upon choosing a date format, i get the following error

Even if i remove the only column from the excel file and try again, the first error does not come up but ehrn i submit, the second error occurs.
