Lines included in Form NOT Displaying as Column Titles

I created a database in Zoho Creator. I created the line items for the input screen. All of the line items included on the input capture screen are NOT displaying as column titles in the view report screen.

The line item titles that are not displaying as column titles are also not being included in the filter lists.

The column titles on the view report screen are not in the same sequence as on the input capture screen. I can grab the column title and move it right or left to the right position, but this does not seem to make any difference.

I'm not sure what I am doing wrong.

Do you have instructions for how to set up the database?
