Link Two Forms then Return

I am very new at this and just starting to design a Form. (Scripting is a bit of a nightmare at this stage!)

What I would like to do is in Form A have a dropdown which says "Do you have Additional addresses". (Yes/No) If the answer is "Yes" then the client will be taken to another Form B where there are fields for adding this information. Once that has been input then I would like the client to be able to return to Form A to complete the rest of the Form. If "No" then nothing will happen

Is this possible to achieve or is there a simpler way of capturing this information on Form A itself? In any case, when information is submitted will I be able to see all the addresses entered?
