Linking Applications

Hi, I have created one application that tracks the medicines and its dosages and manufacturers. I have also created another application for patients names and the frequency of their medication. However, I wanted to know IF it is possible that the medicine field gets its source from the previous application's medicines field?

Any help in this regard would be great!
