Log in

I am trying to post a survey on my blog without having the visitors log in and I tried to pick the code which says that it doesn't require log in details but when I pasted it on to the blog, it still asks for the log in.

Moreover the code doesn't change anyway at all between log in and without log in optinos.

Please help
