How do I get the data ie: Name to show up in my lookup field instead of the internal ID?  I have successfully set up my email notification but I can't figure out how to insert the Name of my customer instead of the internally generated ID.

All of the other fields are displayed correctly except it is showing the form lookup field internal id instead of the actual field data.
