multi-part form

Can this tool allow you to create multi-part forms?

for instance a general section and detail list:

name:
date:

month kWHrs Gallons Therms
month1 xxx xxx xxx
month2 xxx xxx xxx
month3 xxx xxx xxx

I imagine it would have to be two backend tables tied together with a primary key. Also, can you export data in CSV format?
