Multiple Forms For One View in Creator

I am working with the CRM-mini application in Creator and want to build a second form that feeds into the View Leads view in the Leads tab.

The second form will only have 4 fields of data and will be entered on a public website, but I want to keep the form that is in the CRM application already for use by me or my group.

Is it possible to have two different forms feed into a single view, and can the forms have some different fields in each form?

I have searched the forums, but think my keywords were not correct as I did not find anything to that affect.
