See dvdborrowing.blogspot.com for the form that I am having issues with.

When users fill out the form and click on "submit," all that reaches my e-mail is the sentence, "I wish to borrow a DVD."

How do I configure the script to get the rest of the form info to show? (e.g., their name, dvd they wish to borrow, etc).

I am a novice at scripting, and need a little step-by-step help.  (I read about scripting in the wiki section, but could not find examples specific to my need).
