Notes from one module to another

I have a module called "Potentials" which when the status changed to "Closed-Won" will be automatically converted in to a "Patient" (Custom module) using a custom function. I need to move the notes from the "Potentials" record to be moved to "Patient" when a new Patient is created.

How could I achieve this?
