Printing a form

Hello.
I have an application for ministry leaders in our church to fill out a form for ministry events. I have the form created for their input. What I'd like to be able to do is have a section below their input fields for office use only for approval signatures, etc. I'd also like for them to be able to print the form on clicking the submit button. Is this possible?
