Problem adding single select

Problem adding single select

I am having a re-occuring problem adding a single select (Pick List, radio) to my application. In Firefox, I get "error has occurred, kindly reload"; in IE I either get a javascript error which refers to a missing object or the page just hangs.

Please help!
