Problem with embedding forms on website

Hello,

I have a problem with embedding ZOHO forms on my website.

I created a contact form where people can fill out their name, phone, email, etc. Then on the website it says:

Sign In
Username:
Password:

And then after I sign in with my ZOHO info, the form shows up. Obviously this is not going to work out if I want anyone to be able to see and fill out the form.

How can I solve this problem?
