Public Forms - Still not working

I did send teh request from the form and received confirmation went back in and copied and pasted the code and that is when the problem began that when I logged off of zoho the form was no longer publicly accessible online. The answer below does not help me because I already followed every step your website provided. Is the problem because I have a free account or because I toggled between two accounts on one computer? I cannot see the form on another computer either. The form is here: http://www.stormhavendesigns.com/MBN/edit/groups_form.htm .
