Radio Buttons

The radio buttons in the form do not seem to be working now. My web page is: http://gomahsmustangs2008.com/?page_id=24 and I am using the survey buttons to give the user the ability to respond reagarding planning to attend our high school reunion. I received 24 responses to the form. Now that the buttons do not work, the form is not accepted (I made the buttons a must press when I created the form).

I would appreciate any help I could get on fixing this. Thanks very much.
