record gets created in two sections

I have a form called "National Chapters" with two entries "region" and "chapter". I have another form where a new member has to fill out and one of the questions is which region and which chapter they want to join.
my question is: Is there a way using the lookup to first choose the region and then based on that selection they get the lookup for the chapter.

Also when the new member chooses the chapter that they want, that record gets created in two sections the section of the chapter and also the section for all the members
