Need help here for repetative data from lookup field, The details are below

I want to achieve as Country >> State >> City 

Form 1 :Created form and added country in a table

Form 2 : Created another form and added lookup for country field and text field state to capture state

now the problem is one state belong to many cities, so i have to create another form to add state as lookup field and city as text field.

If I add the city along with state in form 2 and want to add municipality and zone as well for the city, have to create 2 more forms and pull the data for lookup field as above, If I don't do this and fetch the data in lookup field the State will repeat number of time = city as in report, and same will happen if i want to add other data against same city / state.

My question is here that any possible way to avoid repetation of data in Look Field through coding / function ? Can someone help.
