scan forms overlay input fields, boxes, etc using ZOHOCREATO

I am curious to know if I can scan a form as a background then overlay input boxe, check boxes, drop down, etc. then I need to print the background and the input overlay properly aligned and proportional. for example, a long tax form, or a company employee application.
