Script has stopped working

For Support:

I added selections to some lookup fields in my "Farm-Based Education Material Database" and now when I go back to the "Search with Options" form, the "on user input" script for the "Material Sub-Categories' is not working. The script is there and I can see it, but in the actual form, it shows no evidence of detecting the script (ex. no little arrows turns).
I would expect the arrow turning even if there was an issue with the script, but there should be no issue as i changed nothing with the script and it has works for two months.

Thank you for addressing this immediately. I have a customer waiting to start using this form for general use this week.
