Sending url...

I have two web pages selling different products. I have a user submission form created with zoho creator and I would like to use the same form on both pages and capture the referring url information into the same database (a url field that would show which web page the submission came from). I know that I could simply create the same database twice but would like both web pages info in one database. Is this possible?
