size limits?

Hello, we've made a 135 question survey application for our church. When we try to embed it in our html, half or sometimes none of the questions appear on the page at all? We have no real background in this at all? the application works great, but getting it to show in a web page is the issue. we've tried the permalink, which seems to work, but not sure if that's the proper way to go.
We've shared access with ZOHO. The application is "Church Health Survey.
Quite honestly we're not 100% sure this is the best method for us to use. but it's working so well. Just a matter of getting it on a web page.. We thought about cutting the size down, but not sure how to make the pages flow and keep the data connected thru the vaious pages till they hit the submit button?
please advise..
