Static form, users change same data

I am trying to create a form where the data that is input stays for all users. For instance if user A checks a box, when user B opens the form the box is then checked. We would also need that info tracked, so if a user changes that box it would also track that information.

I am a newbie here and I'm sure I could do it with code but I wasn't sure if I'm missing some easier way to do it than coding it. I looked around on the forums and couldn't find anything or even think of what to call it to search for it...

Any help is appreciated...
