Submitted data into email?

Is there a way to put submitted user data into an automated email?

For example: My user completes a form - Name address and phone.

That information is logged into the database.

Now the notification email goes out to the sender with...say and automated agreement.
IE.

Agreement Copy:

Thank you {FULL_NAME} for your submission. You agree have the items shipped to {ADDRESS} and in the event of an emergency, you can be reached at {PHONE}
