Submitting forms

A script has been added to our form so that when the submit button is selected, a confirmation email should be sent to the user as well as the admin email. But for some reason it is only working sporadically. Sometimes an email is sent to the user and admin emails and sometimes it isn't.

Is there some way to correct this problem?
