Trouble closing the deal

I have created my form and my form results view. Now the problems come in taking them to my web site and making them work together.

When I submit the form, the form data doesn't get to the form view on my web site. It does get to the form view here on Zoho's web site.

Also, after the user clicks 'Submit' the "Your reservation is confirmed' statement flies by so quickly that it is unreadable. Also, I want to take the submitter to a 'thank you' page but I haven't been able to accomplish that either. The instruction here on Zoho are not clear to me.

I changed the color of my form border and the header bar but the change didn't hold?

I know this is a lot but can anyone help with these issues?
