URGENT HELP NEEDED: Can't Add Data to Database and Can't Update Deluge Names

I have two problems:

1) Member Data Form: I am unable to submit a form I created to the database called 'Member Data'.  All other forms seems to add data to the database as purposed. Please help, this is critical.

2) Approval Decision Report (View): In the Approval Decision Report View, there are two fields called 'Registration'. One should be called 'First Name', the other 'Last Name'.  I have checked the fields many times to ensure that they are set up appropriately (and they are)--both the Label and Deluge Names in the actual fields located on the 'Approval Decision' Form. I believe I could fix the problem but I am unable to determine how to 'open' the actual deluge script using the Free Flow Scripting link to change the field names from Registration to First_Name and Last_Name. It is important to note that I have set up the Deluge Names in the fields located on the Approval Decision Form as  First_Name and Last_Name but it is not working.
